Mukhyamantri Jan Arogya Yojana was launched in September 2019 by the Government of Uttar Pradesh as a state-level health insurance scheme to cover families that are not included in the central Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PMJAY).

Under this scheme, eligible families get cashless treatment up to ₹5 lakh per year at empanelled government and private hospitals. The scheme covers 1,393+ medical packages.

  • Permanent resident of Uttar Pradesh

  • Must possess UP ration card or domicile certificate

Check eligibility at the nearest Common Service Centre (CSC) or government hospital

No cash payment needed up to ₹5 lakh per year
